Documents for Affiliation
• Trust & NGO Details.
• Society Registration copy.
• Trust Pan Card.
• Account Details of Society Person.
• Stamp of 100 Rs.
• Hospital Association letter.
• Faculty Academic Qualification And Minimum 2 years experience Details. ( Bachelor In BMLT / BOTT / BSC Nursing Etc )
• Aadhar Card & Pan Card of owner.
• Rent agreement of Institute on rent otherwise Deed of institute.
• Affiliation form filled by Owner. 11- Photos of Training Centre.
• Fire NOC
Affiliation fee
• To start a Allied And Healthcare college you should pay 50,000/- Rs. for run any of one approved Allied And Healthcare course, approved courses list given in course section.
• If any Allied And Healthcare college want to run more than one Allied And Healthcare courses they should pay 25000/-Rs. more for each courses.
Verification fee
Verification fee is 25000/- Rs. it will be paid before physical verification or at verification time.
Annual Renewal Fee
All the affiliated colleges need to pay the annual renewal fees of 15000/- Rs after the completion of one year from the date of affiliation. It is mandatory to pay this fees to keep the college website up and further on-going processes.
Mode of Payment
Council accept all type of payment mode like Cash, Cheque, Demand Draft, Net banking.
Number of Seat
• 40 (forty) seat are approved for each Allied And Healthcare courses if any Allied And Healthcare college want seat enhancement they should pay 500/- Rs. for each seat.
Norms for Affiliation
Any registered society organization trust can run the entire courses of State Allied And Healthcare Skill And Training Council Uttar Pradesh. But no Institute or individual shall do it on commercial or business ground.

• Constructed Area 2000 Square Feet
• Principal Office 1
• Clerical Office 1
• Staff Room 1
• Library 1
• Laboratory 1 ( 300 sq. Ft. with all required appliances / stuments )
• Lecture Room 3 ( 400 Sq. Ft. Minimum )
• Separate Toilet for boys and girls
• First Aid Room
Facilities Required
• Parking
• Drinking Water
• Ramp for Handicap Students ( Optional )
Note :
• To provide the practical training to student’s hospital affiliation required.
• To provide the modern education audio visual devices required.
To get the affiliation any welfare society can apply with full bio data with building agreement and required documents then the inspection team of the Council will visit your centre and submit the report to the Council for permanent affiliation. The Council as per the norms will charge the verification and affiliation fees.
Note :
• Once paid fees will not be refundable or adjustable in any circumstances.
Staff Required
• Any Allied Healthcare Post graduate with 5 to 6 years experience should be the principal of institute
• Medical lab technician
• Clerk
• Lecturer
• Computer operator
• Sweeper
• Peon
Other Infrastructure to run the institute required as per the norms of State Allied And Healthcare Skill And Training Council.
